rotation description
This direct patient care rotation with Community Virtual Care (a branch of Community Health Services)
is a great opportunity to provide direct patient care in a unique format (by phone and video).
The patients are enrolled with the CVC program for either chronic disease management (education
and self-management) or palliative care support. The current disease states supported under the
program are COPD, CHF, diabetes, HTN, CKD, and soon, liver disease. Each patient is supported
through these programs with an RN and the RN will refer the patient to the CVC pharmacist to
either receive more education about their medications or to address specific questions such as
potential side effects, polypharmacy, natural products, etc. Important philosophies that
will be emphasized include patient-centeredness, evidence based practice and collaboration
with other health care professionals.
The resident will work from home during this rotation and connect with patients and preceptor via phone, Teams, and zoom. The CVC team is comprised of over 20 health care professionals including pharmacist, registered nurses, social workers, a dietician, and diabetes nurse educators. The resident will meet with the team during informal huddles, weekly rounds, and may be asked to present a patient case and/or provide inservice to nurses for a specific topic.
