Criteria for completion of program

Residents need to successfully complete and pass all rotations and requirements of the program curriculum. The one45 form below serves as the checklist for completion of the residency program and is completed by the Residency Coordinator when determining if the resident has met criteria for program graduation and awarding of the “Accredited Canadian Pharmacy Resident” (ACPR) designation.
