Med safety MUM MUS PharmInformatics
The resident and preceptor shall complete and review together the final evaluations. The mid-point evaluation should be completed at the half-way point of the rotation. Final evaluations shall be completed at the end of the rotation.
There is no midpoint evaluation for rotations less than four weeks in duration.
Mid-point evaluations:
• midpoint for leadership-management is done in late February or early March
• midpoint for project is usually done in January
• midpoints are not done for rotations of 3 weeks or less in duration
Final evaluations:
• resident’s self-evaluation
• preceptor’s evaluation of the resident
• resident’s evaluation of the rotation and preceptor
• residency coordinator evaluation of pass or fail status for non-DPC rotation
Other Evaluations:
• evaluation of scholarly writing skills is done for medication use management (MUM) rotation
• preceptor self-evaluation and reflection done annually
