Abstract
Transitions of care represent a vulnerable time point during which patients are at an increased risk of medication errors. For example, adverse drug events and unintended medication changes following discharge can lead to medication-related problems which are one of the main contributors to hospital readmissions. Pharmacists play a vital role in preventing these problems during transitions of care. The purpose of this study is to characterize hospital pharmacists’ current involvement at discharge, identify which discharge interventions pharmacists believe should be prioritized and which healthcare team member should provide them as well as establish strategies to optimize the discharge process.
