Abstract
Hospital at Home (HaH) is an innovative model of acute care that was incorporated into Island Health Authority in November 2020. Through a combination of in-person and virtual supports, patients are able to receive safe and effective care from acute care healthcare providers from the comfort of their own home. The program functions as an additional medicine ward of the Victoria General Hospital (VGH) and Royal Jubilee Hospital (RJH). This study assesses the impact of the clinical pharmacist within the HaH care delivery model, using a combination of quantitative and qualitative indicators.
