Abstract
Medication errors (ME) can cause direct harm to patients and have implications to healthcare spending. Paediatric patients have up to a three times greater risk of medication errors compared to adults. Opioid related medication errors are the most common cause of harm within the paediatric population. In an ongoing effort to prevent opioid ME in paediatrics, we performed a retrospective mixed methods analysis of Patient Safely Learning System (PSLS) reports at BC Children’s Hospital to identify the types of errors and contributing factors.
