Abstract
Hospital discharge is a critical transition period for geriatric patients, who typically have multiple comorbidities and medications requiring follow-up care in the community. Previous literature has characterized a disconnect in communication between inpatient and community providers. At Surrey Memorial Hospital (SMH), there is currently no standardized discharge process and communication throughout transition of care can vary based on staffing and timing. Given the limited research surrounding processes within the geriatric population, we conducted a survey to characterize the perceptions, attitudes, and experiences of discharge communication amongst geriatric patients and healthcare providers in Surrey.
