Characterization of Empiric Vancomycin Dosing in Obese Patients on Intermittent Hemodialysis in the Fraser Health Renal Program (EVDO-IHD)

Abstract

Hemodialysis patients are highly vulnerable to infections and vancomycin is commonly prescribed to treat gram positive infections. ASHP/IDSA 2020 Guidelines recommend maintaining pre-dialysis vancomycin concentrations of 15-20mg/L for serious infections, but there is currently no consensus for optimal empiric dosing strategies in patients who are obese and have end stage renal disease requiring intermittent hemodialysis. The purpose of this study is to characterize local vancomycin dosing strategies in this population and their ability to attain target trough concentrations with hopes of providing insight on whether changes are needed to result in improved patient outcomes.
