Assessing Vancomycin-associated Acute Kidney Injury (AKI) at Burnaby Hospital and Surrey Memorial Hospital

Abstract
IDSA guideline now recommends using AUC/MIC to monitor vancomycin therapy rather than targeting trough level of 15-20 mg/L due to the lack of clinical benefit and increase AKI risk associated with higher trough level. Canada has not adopted this change due to literature inconsistency behind AUC monitoring and high variability and logistic concern in the Bayesian software that is needed. In order to review the safety of using trough monitoring, we conducted a retrospective chart review for two Fraser Health sites to assess local vancomycin-associated AKI rates and to characterize AKI risk factors including vancomycin trough levels.
