Resident Not Meeting Performance Expectations
Preceptors may determine that a resident is not meeting the performance expectations
of the rotation well before the midpoint or the final assessment ITAR.
Preceptors are encouraged to notify the residency coordinator early in the rotation if they have concerns that the resident is not meeting expectations. Once notified the residency coordinator can take steps to help support the resident.
The purpose of this form is to document the performance issue so the residency coordinator and preceptor can take action to ensure the success of the resident.
When preceptors identify that a resident is not meeting expectation the residency coordinator will forward the one45 form below titled "Residency Coordinator Notification by a Preceptor of a Resident Not Meeting Expectations"
